PLEASE READ CAREFULLY – YOU MUST AGREE TO THE FOLLOWING:

* I hereby certify that to the best of my knowledge, this information is correct.
* I am not under the influence of any drugs or alcohol.
* I agree that my tattoo is correctly drawn to my specifications and allow for artist interpretation.
* I understand that the tattoo is permanent.
* I understand that there is the possibility of infection and/or allergic reaction. I agree to follow the instructions I was given for the care of my tattoo.
* All questions have been answered to my satisfaction.
* I agree to immediately notify the artist the moment I feel dizzy, light-headed, or faint before, during, or after the procedure.
FAILURE TO DO SO RELEASES HELEN TATTOO CO. AND THE ARTISTS OF ALL RESPONSIBILITY.
* I hereby release Helen Tattoo Co. and the artist of all responsibility for said tattoo.
* I agree to allow my blood to be tested in the event my artist becomes exposed via puncture, scratch, etc. The information I release will be confidential.
* I am voluntarily obtaining services of my own free will and volition.
* I have had the opportunity to read and understand the documents presented to me.
* I have received written and verbal aftercare for my procedure.
